Patient & Consent Forms

Prior to your first appointment, please proceed with the following:

• Download or duplicate the forms relevant to you (To duplicate, when in the Google doc, click File > Make a copy)

• ​Fill out the forms with your personal information

• Return the required forms via email to amymarcus1019@gmail.com

  • Note: If you duplicated and filled out a Google doc, use the blue button on the top right of the Google doc that says "Share"​
